The Ghana Football Association has requested that all Premier, Division One and National teams should provide the underlisted particulars of their Coach.

• Name • Date of Birth • Coaching License(s) with dates and photocopies (inclusive) • Contacts (Cell Number and Email address) • Nationality • Passport, National ID/Voters ID for verification. Clubs should notify their respective coaches to submit the following additions;

• CV • Philosophy (Coaching) • Strategic Plan

Kindly note that, these information must reach the Technical Directorate of the GFA by close of day on 30th October, 2015.
