Division Clubs are kindly being requested to furnish the following information.

1. Declaration of colours (Descriptive with pictorials)

2. Venue for matches

3. Authorised signatories

4. Submission of Club Licensing forms

5. Submission of Certificate of incor portion

6. Payment of Affiliation fee GHC500

Clubs are reminded that deadline for submission of Club Licensing forms is on Monday January 11, 2016.

We wish to remind you of Article 37 (19) of the GFA General Regulations which states inter alia " All official correspondence including but not limited to daily and weekly information of the Association, it's bodies, it's communiques or the Secretariat' shall be deemed to have been duly served on a clue, it's officers, players, supporters, agents, representatives or whoever described, of the service of the said correspondence is communicated to the club or the Secretary or the club owner, or the club representatives submitted to the GFA through Fax, e-mail address, postal address, or delivery of a hard copy thereof or is made or published through the GFA newspaper or news letter or other national newspapers, the GFA website or announced through any radio station or television station or posted on the GFA notice board."
